I give permission for my child/children to participate in Judy Highfill's Art Program

In case of an accident, I grant permission for my child/children to receive medical treatment, if needed and authorize the attending medical personnel to administer any necessary medical attention.

I give permission to use my child's/children's PHOTO in promotional material; printed or online. I understand that my child's name will not be included.

I give permission to use my child's/children's ART WORK in promotional material; printed or online. I understand that my child's name will not be included.

Your spot is not held until payment is received.

Payment is to include class fee plus Pay Pal.

After your Registration form is received, I will send a confirmation email with payment instructions.

Registration is complete once payment is received.
